Care Coordinator, Licensed Practical Nurse (LPN)/Registered Nurse (RN)

Location: Hybrid - within Grand Rapids, Michigan area
Working Hours: Full-time, salary, 40 hours per week
Reports to: Director of Operations
Salary Range: $57,000 - $65,000 per year based on experience
The Role:
Root Functional Medicine is a pioneering organization in functional medicine, dedicated to personalized patient care and innovative health solutions. We are seeking a proactive and organized Clinical Coordinator to ensure smooth clinical operations and exceptional member support across multiple departments.
What You’ll Do:
Customer Service Management
- Manage member phone lines and respond promptly to inquiries.
- Monitor and manage help desk for timely responses to sales and support requests.
- Handle portal messages for both customer service and provider support.
- Provide excellent onboarding experience for all new members.
- Guide members to various tiers of membership.
Clinical Administration
- Work collaboratively with doctors and dietitians to ensure excellent quality care.
- Facilitate lab ordering and upload test results accurately.
- Manage scheduling efficiently.
- Audit dietitian panels and manage inactive member statuses.
- Ensure forms are checked and sent promptly.
- Complete Prior Authorizations.
Corporate Account Engagement
- Proactively reach out to increase level of engagement.
- Track key indicators for company success (i.e lab and symptom improvements).
- Work with operations to organize and report on key indicators.
Software and Documentation Management
- Troubleshoot software issues for customers and providers.
- Maintain and update shared documents and databases.
- Create and maintain SOPs related to clinical and member support
Other duties as assigned - must be willing to grow with the role as we are a fast growing organization!
Qualifications:
- Licensed Practical Nurse (LPN) or Registered Nurse (RN)
- Proven experience in customer service and clinical support roles within a healthcare setting.
- Familiarity with electronic health records (EHR) systems and scheduling software.
- Excellent organizational skills with the ability to prioritize tasks effectively.
- Strong interpersonal and communication skills, both written and verbal.
- Knowledge of medical terminology and HIPAA regulations.

Product Manager
Join us at Root Functional Medicine as a dedicated Product Manager to spearhead the development and execution of products that enhance health and wellness through technology. In this critical role, you will oversee the product lifecycle from concept to launch, ensuring that our digital solutions are accessible, efficient, and personalized. Your strategic vision will guide the creation of products that leverage our technology platform to transform healthcare experiences. Location: Remote, located in the United States
Working Hours: Full-time, 40 hours per week
Reports to: Founding Software Engineer
Salary Range: $110,000 - $130,000/yr. based on experience
What You’ll Do:
- Strategy & Vision
- Define the product strategy and roadmap, aligning with the company's mission to innovate healthcare solutions.
- Collaborate with cross-functional teams, including engineering, design, and leadership, to envision and prioritize transformative products.
- Analyze market trends, customer feedback, and competitive insights to identify opportunities for new features and products.
- Execution & Leadership
- Lead the product development process, ensuring that projects are delivered on time and meet quality standards.
- Act as a bridge between technical teams and non-technical stakeholders, effectively communicating product features and benefits.
- Foster a culture of innovation and continuous improvement, encouraging feedback and ideas from all team members.
- Product Development & Integration
- Oversee the integration of essential services and APIs, such as payment gateways and scheduling tools, to enhance product functionality.
- Work closely with the engineering team to ensure that product development aligns with technical capabilities and business goals.
- Champion the adoption of best practices in product management and agile methodologies.
- Quality Assurance & Compliance
- Ensure products meet industry standards and compliance, particularly in healthcare settings.
- Lead user testing and feedback sessions to refine products and ensure they meet customer needs.
- Monitor product performance post-launch, implementing improvements based on user engagement and satisfaction.
Qualifications:
- Proven experience as a Product Manager or similar role with a track record of successfully managing end-to-end product development.
- Strong understanding of the healthcare industry, technology platforms, and digital product ecosystems.
- Excellent organizational and project management skills, with a demonstrated ability to lead projects and meet deadlines.
- Outstanding communication and interpersonal skills, capable of motivating teams and engaging with stakeholders at all levels.
Preferred Qualifications:
- Bachelor’s or Master's degree in Business Administration, Health Administration, or a related field.
- Experience with regulatory standards relevant to healthcare technology, such as HIPAA and HL7.
- Previous exposure to healthcare application development and a passion for functional medicine.
